Along with the SpamBlacklist, ConfirmEdit is an essential extension for dealing with spam. It presents users with a captcha if they try to make carry out a certain action. The actions are configurable, but can include page creation, addition of URLs, and account creation. You can choose to exclude certain user groups from seeing the captcha.
